-Create a 30- or 60-second video on what it means for them to be the first tobacco-free generation.
-After-school programs, youth enrichment programs and other youth-focused nonprofit organizations are invited to have their youth submit videos through Wednesday, August 2. If their video is chosen, the organization will receive $5,000.
-Rhode Island youth are also invited to submit a video on their own through Wednesday, August 2. If their video is chosen, a youth-focused non-profit of their choice will receive $5,000.
-Between August 11 and August 23, 2017 the general public will then be able to vote for
their favorite PSA video via voting buttons on www.cvshealth.com/communityheroes
The organization that receives the most votes by 5:00 pm EDT on August 23, 2017 will
receive a bonus $5,000 grant.
-Verified 501(c) (3) tax status.
-Youth participants must be at least 12 years old to participate. All participants much sign a
waiver allowing CVS Health and participating partners to use their video at will. Participants
under the age of 18 must have their parent or guardian sign the waiver.
